I need this form converted into a fillable PDF, where customers can fill in the required info, then save/print/email. There are number of revisions that also need to be made & they are as follows:
1. In top section (customer info), change order of department boxes to: OR, SPD, BIOMED, PUL, GI, OTHER & have this be the last line in the section.
2. Change cost center # to email.
3. Order for the top section is as follows (left to right, up to down): Contact Name:/Facility Name:, Telephone #:/Email:, Purchase Order #:/Date:.
3. Change instrument/equipment information #1 to "REPAIR #1, REPAIR #2, etc.
4. Remove the check box w/ "I have questions..." in order to make room for Shipping Instructions section.
5. Remove very last line on page "Please call..."
6. Add "SHIPPING INSTRUCTIONS" w/ the following info w/ a check box next to each option as bottom section:
