I have a commercial questionaire used in my business that I have a scanned copy of from a book. I'd like to have them retyped & saved in an Adobe format to where I can email them & the customer can fill in the questions without printing it off. I'd like to have a back up version of these in excel, in the event I need to add or delete questions off the questionaire in the future. In addition, I'd like to add more info on the initial page to include a line for customer contact info i.e. phone, email, website, mailing address, FEIN. Can we clean this format up to where we have more space to put in comments. Include our agency name and logo @ the top of the page. Thanks I have a $20 budget for form.
