Solution planning support for Microsoft Dynamics CRM project
A solution architect is needed to assist with planning the development of a platform to support healthcare service coordination using Microsoft Dynamics CRM. Please respond with your relevant experience, rate, available start date and the weekly amount of time you can dedicate to this project.
The objective of this project is to develop a platform to support collaboration and care coordination amongst various healthcare and community service organizations in large metropolitan areas. Members of the community will include primary care physicians, secondary care specialists, behavioral health practices and services, transportation service providers, physical therapy service providers, dietary service providers, school-based programs, etc.
Patients with complex cases of several chronic diseases will be referred by their primary-care or insurance provider into care coordination programs which involve a combination of multiple services by different providers. The care coordination program will help ensure that the right care is given, in a timely manner and that all actions are followed through for better health results and ultimately lowering costs to the system.
During the delivery of services, many actions and events will occur which need to be communicated to members of the patient's care team. Based on which event has occurred, some followup actions may be required by members of the care team. The platform will provide tools to configure the supporting coordination workflows and notification rules.
The basic process flow is:
• Patient is referred to care coordination program
• A care coordinator conducts an assessment, defines goals and assigns actions to service providers in the care team
• Care team members provide feedback on actions in the plan
• Care team members can view relevant information in the patient's case file
• Some events may cause an alert and specific followup actions to be raised
This platform will be developed on Microsoft Dynamics CRM, and will initially be a cloud-based solution.
