PHP/AJAX Enhancements for tourn_maint.php

Under the Overview section, there's a button/link for "Register Archer for Tournament". The work that you did before on the AJAX dropdowns needs to be called from this link/button. I have altered your work slightly, so for this work, please reference:
Basically, what I modified is as follows:
I needed more information returned from the archer name lookup, so I added a few fields to the page. I also got rid of the "position" radio buttons and instead am using the lane dropdown to contain the lane and position.
So, what I'd like to have happen is to have a popup form/dialog that is called when the button is clicked. Inside the popup dialog/form, present the fields just like you did for the original page. Once the user has filled everything out, the user will click the "register archer" button and this will call the code at:
<form action="reg_archer.php" method="post"> All of the code necessary to register the archer is in place and is working, so no changes need be made to this.
Once the archer has been added/registered, display a message that says "archer XXXXX has been registered!" and then return the user to the tourn_maint.php page and then refresh the page to reflect the registration.
