I am a new specialist signing up with insurance companies. You will need to call each of the companies and ask for a copy of the application to be credentialed with the insurance company. I need them in a pdf format. First ask them to have it emailed. If that is not possible then have them fax it. They may try to tell you the only way to send it is by mail, but with a little convincing they will send it via email or fax.
