Using COBOL-Form, create a standardized form that looks like the scanned document. Each form takes 5-8 hours to create. I have been doing it myself - it is easy but time consuming (and very boring). We have around 50 forms that still need to be created, as soon as possible. The document file includes the editing software. Please quote a price for a single form, and tell me how many you are willing and available to create immediately. Priority will go to people who can do many of them, quickly.
