Purpose of Job: Figure out how many miles I have driven to various doctors since 12-26-2008 and complete attached medical mileage form for all visits. This task involves four steps: 1) If hired, you would reach out to a list of approximately 20 doctors and healthcare sites I provide and request that they provide you with a list of all my previous doctor appointments/visits at that location. You may need need to obtain their fax number or scan and send a release, on my behalf, to them. 2) Each medical provider will then provide you a list of appointment dates since 12-26-2008. Sometimes working with freelancers not on Elance, I decided to create an account with WebMoney. But being in Canada, I can't use my Canadian credit card to send money in my WMZ purse or my WMR purse.
