Attached is a zip file with all the bits I think you should need. Basically I've set the front and back of the questionnaire in illustrator (i've sent you the pdf files of this) and it needs to be done in word. The document size has to be A5 2 pages. Where the form is on the reverse it needs to have interactive fields. ie where the tick boxes are I need the person filling it in to be able to click in the box and it will automatically tick. I also need the comments box at the bottom to have a text field in so that when they click in the box they can just start typing. We have to use exactly the same fonts as I've used (fs albert in zip folder) in the visual and the same colours, as these are corporate colours.
