I have a "info card" that I need reduced to Credit card size. I need to be able to upload this into vista print or send to any business card printer and have them print directly on to credit card size. I need to keep the Assurant Employee Benefits logo on the card. I need to have space for the "Policy Holder" number that is fillable. I need to have a space for the "member number" that is fillable.
